Abstract

The landscape of cardiovascular medicine has undergone a seismic shift over the last century, transitioning from high-risk exploratory surgeries to ultra-precision, minimally invasive interventions. This chapter provides a comprehensive chronological analysis of the milestones that defined modern cardiology, beginning with the foundational era of open-heart surgery and the pivotal invention of the cardiopulmonary bypass machine. It explores the 1977 revolutionary breakthrough by Andreas Grüntzig, whose first successful balloon angioplasty challenged the necessity of the surgical scalpel and birthed the field of interventional cardiology. The narrative further examines the "Stent Revolution," detailing the transition from Bare-Metal Stents to the sophisticated Drug-Eluting Stents (DES) that effectively addressed the challenges of elastic recoil and neointimal hyperplasia. Beyond coronary arteries, the chapter highlights the transformative impact of Transcatheter Aortic Valve Replacement (TAVR) and edge-to-edge mitral repairs, which have redefined treatment paradigms for structural heart disease in elderly and high-risk populations. Finally, the discussion extends into the "NextGen" era, where the integration of Artificial Intelligence (AI), robotic-assisted procedural platforms, and advanced intravascular imaging like OCT and IVUS are setting new benchmarks for patient safety and clinical outcomes. By synthesizing historical context with cutting-edge technological trends, this chapter establishes a foundational understanding of how cardiac intervention has moved toward a future of "Minimal Access, Maximum Precision," ultimately aiming to reduce morbidity and enhance the global quality of cardiac care
